Hospitalization is the placement of a person in a hospital if he needs medical aid or medical examination. Also this event has taken place, if the woman soon to give birth.
There are several types of admission: Extra and Planned.
Emergency hospitalization is to provide immediate assistance in the hospital according to the testimony of health. To patient was placed in the hospital, he is given direction. It can give the ambulance or the doctor. Necessary to hospitalization of the patient was done in a timely manner. Also, the healing process depends on the right diagnosis. If the patient receives a referral for hospitalization in the clinic, he is given a patient card or an extract from it. On admission via ambulance, the patient is given a cover sheet.
In both cases, supporting medical documentation must contain the following information:
Overview of the latest developments of the patient.
The recommendations of the specialists, if the patient passed the examination.
A List of medical events that were provided to the patient.
Should also be given information about duration of disability
There are cases when patients refuse to go to the hospital. In this case, the doctor is obliged to take their application for refusal of admission. You should know that the responsibility for health takes care of the patient. Information about the purpose for which the person is sent to a medical facility.
Planned hospitalization is the placement of a person in a hospital when indicated. In this case, the preparatory stage is an important part. When the patient has an accurate diagnosis, all measures aimed at its examination, the hospital will be immediately to move to the required procedures. The latter will result in the human body in order.
Er (Emergancy) is the starting point for hospitalization. Here, the doctor determines whether the diagnosis, and makes the final decision on placement in the hospital.
When patients are admitted to the hospital first of all they are received by a nurse on duty at the reception ward. Those patients who are to be hospitalized have already received the direction from the polyclinic.
The nurse on duty fills in patients’ case histories in which she writes down their names, age, place of work, occupation, address and the initial diagnosis made by a doctor at the polyclinic.
Then a doctor on duty examines the hospitalized patients, measures their temperature, and sometimes other studies are carried out (for example, electrocardiography).
In order to clarify the diagnosis, the doctor of the emergency department may invite appropriate specialists (surgeon, gynecologist, neurologist, etc.) for consultation and then gives his instructions what department and wards the patients are to be admitted to.
If the preparatory stage to hospitalization carried out not in full and there is a probability that the diagnosis is wrong, then the hospital will have to spend time for additional examination of the patient and accurate diagnosis. And then move on to treatment.

Taking a Medical History
When patients are admitted to the hospital first of all they must have the case history is given by their physicians or is given a cover sheet from Er.
The medical history or (medical) case history of a patient is information gained by a physician by asking specific questions, either of the patient or of other people who know the person and can give suitable information with the aim of obtaining information useful in formulating a diagnosis and providing medical care to the patient.
Case history should consist of an introduction, that has a few clear and concise opening statements, which typically include information on: Name (pseudonym), Age, Marital status, Occupation, Central problem; history of Presenting Complaint - The impact of the illness on the patient's life, details of previous treatment and include information on who administered management (when and where), what the treatment was (and preferably the dose and duration of treatment), and the patient's responses to treatment; Anamnesis of disease; The result of laboratory and Instrumental methods of diagnosis; The clinical diagnosis: basic disease, its complications with accompanying disease; The diagnostic basis; The Concluding History is important at this point to collect the relevant data about the patient (all positive findings) and construct a logical framework of the case.
The medically relevant complaints reported by the patient or others familiar with the patient are referred to as symptoms, in contrast with clinical signs, which are ascertained by direct examination on the part of medical personnel.
Medical histories vary in their depth and focus.
For example, an ambulance paramedic would typically limit his history to important details, such as name, the reason for admission, history of presenting complaint, health history, surgical history, allergie, current medications, physical examination findings, medical diagnoses.
In contrast, a psychiatric history is frequently lengthy and in depth, as many details about the patient's life are relevant to formulating a management plan for a psychiatric illness. The information obtained in this way, together with the physical examination, enables the physician and other health professionals to form a diagnosis and treatment plan. The treatment plan may then include further investigations to clarify the diagnosis.

CASE FROM ТНЕ PRACTICAL MEDICINE
Today we discussed the case of а 22-years-old white man who was in good health prior to two days ago, when he began to have an abdominal pain. Тhis pain was sporadic and colicky in nature. It began in the epigastrium and has since migrated to the right lower quadrant. Тhе patient has had three episodes of vomiting associated with the pain. Не has been anorectic and feverish. Не has no bowel movements for two days. Не reported no diarrhea, coughing with expectoration or short of breath.
Не has no past history or family history of abdominal pain or any other disease.
Тhе pertinent physical findings are related to the abdomen. Тhere is extreme tendeness to palpation, especially over McBumey's point, Guarding, muscle rigidity and rebound tendeness are аll present. Bowel sounds are absent. Тhere is а difference between the axillary and the rectal temperature. Нis urinalysis, hemoglobin and hematocrit are within normal limits. Never the less, both white blооd count and red rate are elevated. His chest film is clear, but in the abdominal film we observed the psoas line absent.
We established the differential diagnosis with acute pancreatitis, acute cholecystitis [əˈkjuːt kəʊlɪsɪsˈtaɪtɪs], myocardial infarction, gastroduodenal ulcer and perforation of an ulcer, finally, we decided the definitive diagnosis is acute appendicitis. Among the possible complications to consider are perforation, necrosis peritonitis. Тherefore, the prognosis is anceps. Тhе only possible treatment is surgical: appendectomy.
